What clinic management software actually costs in India

On the prices vendors publish themselves, clinic software in India runs from free at one end to Rs 3,00,000 as a single one time figure at the other, and the cheapest published recurring plan here is Rs 499 per user a month. The headline is the least useful part of any of them. What decides the bill is the unit it is charged on, the commitment the headline price requires, and the fees sitting beside it.

Every competitor number below was read off the page named in the source list on 10 September 2026, and each row says whether that page belonged to the vendor or to a marketplace, because the two do not always agree. The Elaior figures come from this product's own source code, and the files are named in the same list.

The price table

Per clinic, per practitioner, per user, per seat

The posts ranking for this query quote a band in rupees per doctor per month. Per doctor is the wrong unit for most of this field. Practo Ray is priced per clinic and caps doctors instead of charging for them, 50 on CM Atom and unlimited on Clinic Management. Clinicea charges per practitioner, so a second doctor doubles the subscription. Insta and DocEngage charge per user rather than per doctor, and Insta enforces a floor of 5 users. Eka charges an extra doctor seat at roughly the whole base plan again.

The same clinic priced on each model: two doctors, two front desk staff, one location, on the cheapest published tier of each. Where a vendor prices per user, the arithmetic counts four logins, because the published unit is the user and not the doctor. If a vendor counts only clinical logins, its number falls.

That order is nothing like the order of the headline prices, and the arithmetic is the vendors' own. It also prices the base plan only, and several of these keep the thing a clinic came for outside it.

What is charged beside the subscription

• A cut of your money. Ray Payment Solution is priced at 2 percent of transactions on CM Atom and 1.8 percent on Clinic Management.
• A fee per new patient. Practo Prime charges the clinic a one time technology fee the first time a patient uses Practo's technology to reach it, valid 6 months. The provider FAQ does not publish the amount.
• Calling. Ray Connect 1000 is Rs 1,499 a month at 1 year, Rs 1,249 at 2 years and Rs 999 at 3 years or more, against list prices of Rs 2,999, Rs 2,499 and Rs 1,999. A 60 second incoming call spends 1 credit, an outgoing one 2.
• The patient portal. Clinicea prices it at Rs 3,999 a month per clinic, which is what its top plan costs for one practitioner and twice what its Starter plan costs.
• Lab and pharmacy metered by staff. Clinicea charges Rs 499 a month per technician and Rs 499 per pharmacist, plus Rs 499 per clinic for white labelled emails.
• Eka Care annual add ons: in clinic pharmacy Rs 10,999, success manager Rs 4,999, website Rs 19,999, Own Your Communication Rs 24,999, form builder Rs 9,999, credits from Rs 500, all stated exclusive of taxes.
• Clinicea one time charges, the clearest published setup fees on any vendor's own page: a customisable EMR at Rs 12,500, Rs 25,000 or Rs 45,000 by form count, plus Remote Self Registration at Rs 14,999 with Rs 4,999 annual hosting from year 2.
• Medixcel charges a setup fee on every tier, from Rs 7,500 on the single clinic plan to Rs 60,000 on the largest inpatient tier.

Who publishes and who does not

• Publishes rupee prices on its own site: Practo for Ray, Insta by Practo for its two standard tiers, Clinicea, Eka Care, DocEngage and Halemind.
• Publishes only through a marketplace: Medixcel and MocDoc. Neither grid is on the vendor's own site, and MocDoc's own pricing URL redirected and then returned 404.
• Publishes no price anywhere checked: KareXpert, whose listing reads price available on request. Practo's hospital facing Insta page shows two tiers with no figures, and the same Insta product priced on the vendor site is price on request on the marketplace.
• Publishes a price you cannot read: HealthPlix, whose pricing page holds its content inside an image rather than as text, so no plan name or figure could be read from it.
• Publishes in a currency that is not the market's: Genamet, in US dollars, while noting that SMS is available only to clients based in India.
• Publishes that the software is free: Bahmni, whose FAQ says the software costs nothing and that anyone may charge for implementation, from hardware advice to rollout and training.
• Claims the crown for it: DocEngage's page says it is the only company in India that published every price. Its grid does carry a figure for every tier it lists, and enterprise above 200 users is quoted on request.

Buying once, and the maintenance nobody prints

Paying once rather than subscribing is still on offer. The published examples are MocDoc HMS Starter at Rs 3,00,000 discounted from Rs 3,50,000, and Insta's HMS Custom tier, described as a one time licence on cloud with the price on request. What no vendor checked here publishes is what maintenance costs from year two. A maintenance percentage is repeated across the ranking posts with no vendor page or tender document behind it that could be found, so no figure for it is printed here. The gap is the finding, and it is a good question to ask.

What happens at renewal

This question has the least published evidence and the largest consequence. The one piece found on a vendor's own property is a post on Practo's blog for doctors dated 15 March 2016: effective 1 April 2016 it was doing a market correction on the prices of Practo Ray and Practo Tab by 25 percent to 50 percent, and existing customers were offered the chance to lock the old price for a tenure of up to 3 years first. A decade old, and still the only vendor published answer found. The commitment ladders say it more quietly: Rs 999 a month on Ray Clinic Management needs a 4 year commitment, and the 1 year price on the same plan is Rs 1,499.

Where Elaior sits, in the same units

Elaior is Rs 1,499 per clinic per month, one plan, billed monthly, with no commitment ladder and no setup fee. That is not the cheapest number on this page: DocEngage starts at Rs 499 per user a month, Halemind Starter is Rs 999 paid annually, and Ray CM Atom matches Rs 999 on a 1 year commitment. What differs is the unit and what sits inside it. The base price covers one doctor, and that inclusion is enforced rather than advertised: the clinic row carries a doctor allowance that defaults to one, and creating a doctor past it is refused with a message naming what the plan covers. Doctors are the only seat metered, so front desk and display logins are not counted. Every add on price sits on the same page as the base price: the pharmacy counter Rs 999 a month, each further doctor Rs 399, AI reading Rs 399, and storage past the included 5 GB at Rs 199 for each 10 GB block.

Elaior's own two meters, stated in the same list rather than in a footnote, because this page asks every other vendor for them: the AI add on includes 1,500 actions a month and charges Rs 3 an action after that, and a part used storage block is billed as a whole block. There is no payment gateway in the product, so no percentage is taken on what the clinic collects and there is no fee per patient. The patient portal, the follow up reminders, the waiting room display and the CSV and xlsx exports are inside the base price, and Clinicea prices a patient portal on its own at Rs 3,999 a month per clinic.

What cannot be claimed: Elaior is new, with no renewal history to point at and no track record to cite. Comparing published prices is fair. Claiming that anyone chose it over the products above would not be.

The questions to ask before you sign

• Is the price per clinic, per doctor, per practitioner, per user or per seat, and what happens to the bill when a second doctor joins?
• What commitment does the headline price require, and what is the price at 1 year?
• Does the monthly figure require paying for a year upfront, and is there a minimum user count that includes my front desk?
• Is there a one time setup or implementation fee, and how much?
• Are the patient portal, the pharmacy counter, the lab, extra locations and an account manager included, or does each carry its own line?
• Is any percentage taken on payments, or any fee charged per new patient?
• If the price is a single one time figure, what does maintenance cost from year two, as a number rather than a percentage band?
• What is the price at renewal, and has the vendor raised prices before?
• What does it cost to export the data and leave, and in what format does it come out?
