WhatsApp follow up reminders without a Business API account
If your clinic runs 1 to 4 doctors and one front desk, you almost certainly do not need a WhatsApp Business Platform account to send follow up reminders. WhatsApp documents the click to chat link itself: wa.me followed by the number in full international format, with an optional message appended as a text parameter. Nothing on that page asks for a Business account, a solution provider or an API. What the free route costs you is automation and receipts. Most of the pages ranking for this question that we read are published by somebody who sells the Platform, so here is the arithmetic they leave out.

The rate nobody can show you
Meta does not display India per message rates on any pricing page we could open. They ship as downloadable rate card files, described there as current and effective July 1, 2026, with a note that the same information is available interactively on the WhatsApp Business website. Neither Meta's developer pricing page nor Twilio's WhatsApp pricing page printed an India figure. Every rupee figure we found came from a provider, and the providers disagree.

The two quotes disagree by about 26 percent on marketing and about 26 percent on utility, same market, same date, and nothing Meta publishes on a readable page settles it. We will not pick one and call it the rate. In any case the per message rate is the wrong number for a clinic this size to plan around. The floor is the platform fee: on the one Indian provider price list we read, Rs 1,215 to Rs 2,500 every month before one reminder goes out. Elaior is Rs 1,499 a month and that price covers one doctor, with each doctor beyond the first at Rs 399, the follow up worklist inside the base price, and nothing billed per message.
Three things going API costs you that the reseller pages skip
- Your number. Meta's documentation says a number already in use with WhatsApp cannot be registered unless it is deleted first. The number patients already message stops being an app number the day you go API. Wati adds, and Meta's page did not state, that the message history is lost and that the number cannot go back to the Business app unless it is deregistered from Cloud API.
- Your wording. Meta's guidelines say templates are reviewed on creation and after any edit, that review can take up to 24 hours, and that a rejected template cannot be sent. Rewording one line for one patient is not something that route offers.
- Your first ceiling. A new Meta business portfolio has a messaging limit of 250, meaning 250 unique phone numbers in a rolling 24 hour period, rising through 2,000, 10,000, 100,000 and unlimited tiers on scaling steps and template quality. For a clinic this size 250 is plenty, and none of the ranking pages we read quoted it.
What Meta's policy asks of you, on either route
Everything in this section is Meta's WhatsApp Business Messaging Policy in Meta's own words. It is a vendor contract term and nothing more, and by its own text it reaches the free WhatsApp Business App as well as the Platform. Four clauses matter to a clinic.
- Consent. You may only contact people on WhatsApp if they have given you their mobile phone number and you have received opt in permission from the recipient confirming that they wish to receive subsequent messages or calls from you.
- No surprises. The policy says do not confuse, deceive, defraud, mislead, spam or surprise people with your communications.
- Regulated Verticals. The policy lists verticals it prohibits buying, selling, promoting or facilitating, and that list includes drugs, whether prescription, recreational or otherwise, and medical and healthcare products. It says you are prohibited from messaging about any Regulated Vertical on the WhatsApp Business App. On the Business Platform it permits limited messaging only in specified countries per vertical, with local law complied with, recipients verified as 18 or over, the required licences and permits held, and no commerce features.
- Health information and sensitive identifiers, with the condition attached: do not use WhatsApp for telemedicine or to send or request any health related information, if applicable regulations prohibit distribution of such information to systems that do not meet heightened requirements to handle it. The policy also prohibits sharing or asking people to share full length payment card numbers, financial account numbers or personal ID card numbers, and prohibits forwarding information from one customer chat to another.
Read the third point twice, because it cuts against both of the slogans on offer. The free app is not automatically the safe choice and the Platform is not automatically the compliant one. We will not tell you that a click to send reminder is exempt from that clause. It is arguable that telling a patient a review is due on Tuesday is not messaging about a medical product, but that reading is ours and not a statement by Meta. Treat the free route as what it is: your clinic's own conversation with a patient who gave you their number, sent by a person at your desk, under the same policy as anything else that clinic sends.
Do reminders actually reduce no shows
The figures in circulation on vendor pages, 25 to 40 percent fewer no shows and patients 25 percent less likely to miss an appointment, arrive with no study attached. The checkable number is smaller. The Cochrane systematic review of mobile phone messaging reminders for attendance at healthcare appointments pooled 22 trials and 9,676 participants: attendance was 67 percent among people sent a text reminder against 61 percent in the control group, a risk ratio of 1.10, with a 95 percent confidence interval of 1.05 to 1.15. Two caveats we would rather state than bury. It measures SMS and not WhatsApp, and it was published in December 2013 as review CD007458.pub3.
What the incumbent charges for the same job
Practo publishes rupee figures for Ray: CM Atom at Rs 999 a month, and Clinic Management at Rs 999 a month on a 4 year option or Rs 1,499 a month on a 1 year option against a struck through Rs 1,999. The plan feature text reads appointment confirmation, reminder and follow up SMS, and no plan on that page names WhatsApp as a channel. Patient communication is a separate purchase: Ray Connect 1000 at Rs 1,499 a month, discounted from Rs 2,999, and Connect 4000 at Rs 2,499 a month, discounted from Rs 4,999. Credits there are consumed by calls, and WhatsApp messaging is not mentioned in that pricing document either.
How Elaior does it, and the three limits that follow
Approving a prescription with a follow up interval files one reminder on the follow up date. The desk works the due list, on its own screen and in the right rail of the queue, and every row carries a wa.me click to send link with the reminder message already written, or a number to call. The outcome is logged either way, reached or could not reach with a reason. The files that implement all of that are named in the source list, so the mechanism is checkable rather than asserted. Three limits follow from the design.
- No delivery receipt and no read receipt come back into the software. What you have is the outcome your desk recorded, not proof the message arrived.
- Nothing goes out while the desk is closed, because a person taps each link and presses send. Reminders wait on a worklist rather than firing on their own.
- There is no SMS and no messaging gateway in the service at all. It was removed on purpose and the patient portal replaced it, where a patient signs in with their phone number and a 4 digit PIN and reads their own records.
Decide it in one minute
- Your desk is open at the hour you would send these, the daily list is short enough for a person to tap through, and patients already message your number: the click to send route is the whole answer, at no cost.
- You need messages that leave with nobody present, or a record of delivery inside your software: that is what a subscription buys, so budget the platform fee first and the per message rate second, get delivery reporting in writing, and reread the October 1, 2026 change above.
- You want both on one number: you cannot have that. Meta says the number has to be deleted from WhatsApp before it can be registered on the Platform.
What this page does not know
- Which template category a clinic follow up reminder falls into. Marketing and utility differ by roughly seven times on the quoted rates, and we did not open Meta's categorisation page.
- Meta's actual India rate. It ships as a download and the two published quotes disagree by about 26 percent.
- Whether the broadcast list limits repeated across secondary sources are right. Meta's own help page would not load, so the number is left out rather than borrowed.
- Whether Meta published the October 1, 2026 rates by its own September 1, 2026 deadline. That date has passed, we found no India figure on any page we could open, and it is the check worth doing before you sign a provider contract.
